Do I need a referral? No referral is needed for NDIS (you just need Capacity Building funding), aged care / Support at Home, private health insurance or self-funded clients. A GP referral is required for Medicare Chronic Disease Management (CDM) plans only — ask your GP about a CDM plan if you have a chronic health condition.
