Aged care & home care packages

HCP · CHSP

Falls prevention, strength and balance, and functional independence for older Australians. We visit homes, residential facilities and community settings — wherever is most comfortable for you.

Medicare (GP referral)

Chronic disease management

Bulk-billing available for eligible clients referred by their GP under a Chronic Disease Management plan. Up to 5 allied health sessions per calendar year — ask your GP to refer you today.

Private health & self-funded

All major funds

We accept all major private health insurers with exercise physiology extras cover. Self-funded options also available — contact us for transparent, upfront pricing with no hidden fees.

Hydrotherapy — South East Melbourne Exercise Physiology

Hydrotherapy uses the properties of water — warmth, buoyancy, and resistance — to make exercise more comfortable and effective, especially for clients with pain, limited mobility, or joint conditions.

Initial assessment

Your exercise physiologist visits you for a thorough assessment of your health, goals, and physical capacity — at your preferred location.

Your personalised program

We design a tailored exercise program and begin regular sessions, with all required reports provided to your funding body.

Ongoing reviews

Regular goal reviews and updated programs ensure you keep progressing. We liaise with your support team, care manager or GP as needed.

Does Exercare come to my home?
Yes — we come to you. Exercare visits clients at home, at local gyms, hydrotherapy pools, parks and community spaces across south-east Melbourne. Telehealth is also available.
Can exercise physiology be funded through the NDIS?
Yes. Exercise physiology can be funded under Capacity Building — Improved Daily Activities in your NDIS plan. Exercare accepts plan-managed and self-managed participants. No referral required.
Can I use my home care package for exercise physiology?
Yes. Exercise physiology is an approved allied health service under Home Care Packages (HCP) and CHSP. We work directly with your care manager and provide all required documentation.
Do I need a referral to see an exercise physiologist?
No referral is needed for NDIS, aged care, private health or self-funded clients. A GP referral is required for Medicare Chronic Disease Management (CDM) plans only.
